The only way for a file to fail to be updated is if the dependencies are
not fully expressed. (Or the system clocks are out of synch and you
have some network file accesses happening.)
For a target time to be simply updated, it means that (1) you have
fingerprints enabled, (2) the target already exists, and (3) apart from
their file timestamps, the stated recipe *ingredients* were unchanged
(implying the target, apart from the timestamp, is up-to-date).
